This form ensures we protect your child's privacy while delivering high-quality sports media. Please complete one per athlete.

1. Image Capture, Usage & Copyright

"I grant permission to photograph and video my child. I acknowledge that the company holds the copyrights to these images. I acknowledge that event photography inherently captures group interactions. Sporty-Clips LLC focuses on consented individuals, but I understand that others may appear incidentally in background or candid shots. I release the company from any claims for compensation or royalties."

2. Secure Gallery Access & Data Privacy

"I authorize Sporty-Clips LLC to host my child’s media on private, unlisted galleries at www.sporty-clips.com, hidden from search engines and AI scrapers. Media is permanently deleted from active servers after three years. To protect all athletes, I agree to keep my access credentials and gallery links confidential and will not share them."

3. Optional: Public Marketing & Social Media Consent

May we use your child's images (without their name) to showcase our work on social media or our website?

4. Optional: Personalized Graphics & Product Details

May we include your child's Full Name, Number, and Team on custom-designed graphics and products?

5. Access & Security Agreement

"I understand that gallery access is for authorized families only. To protect all athletes’ privacy, I agree to keep my login and links confidential. Sporty-Clips LLC is not liable for unauthorized access resulting from shared credentials or third-party circumvention of hosting security."

6. Acknowledgement of Rights & Signature

"I confirm I am the legal guardian of the athlete named above. This consent is valid for 3 years. I may withdraw consent for future use at any time by emailing privacy@sporty-clips.com, though I understand that previously printed or published materials may not be retrievable."
